Students wishing to opt out of the mandatory health insurance must have private insurance that meets the following requirements:
• Must include outpatient benefits
• Deductible and coinsurance/co-payment maximum
is no more that $3,000 for combined deductible
and co-payment/co-insurance maximum for in-
network, no-capped benefits.
• Maximum lifetime benefit must be at least
$100,000; (Graduate Students; benefit must be
$250,000)
• Waiting period for covering pre-existing conditions
must be no more than two years, if pre-existing
conditions are not currently covered.
• The insurance company must have a claims processing office located in the United States
Additional requirements for J1 visa holder:
- Benefit for medical evacuation to home country must
be at least $10,000
- Benefit for repatriation of remains must be at least
$7,500

The 2009-10 Student Health Insurance Online Waiver Form for Winter will be available until December 12th.
