UCR Health Requirement Form


All new students must submit the UCR Health Requirement Form attesting to:

• the fact that they have had the state-mandated Hepatitis B vaccine series or are making progress toward completion of the series, and

• the fact that they have read the Meningicoccal Vaccine Sheet.

• the fact that they have read the Measles, Mumps ,and Rubella Sheet (MMR). 


Read the Meningicoccal Vaccine and Sheet and the MMR Sheet, print the Health Requirement Form, fill it out, sign it and submit it to the Campus Health Center by fax, US mail, scanning and emailing it, or dropping it off.

Note: The Hepatitis B and Meningicoccal Vaccines are available at the Campus Health Center. You are welcome to make an appointment to receive them up to 3 month in advance of your initial enrollment date. Call (951) 827-3031 for pricing information and to make an appointment.
